    Users were confused about where to click to add notes, services, and assessments, especially the small plus buttons and nested subcategories.

    To add to this, a recommendation is that users want the interface to make assessments easier to see from outside the program, since the current view only shows one or two.
    In regards to client assessments page labels, the general assessments tab is confusing because it shows colleagues names instead of the assessment type, forcing users to click each one to find the right assessment.
